One square inch of NET area per square foot of enclosed area, a minimum of TWO openings, and the BOTTOM of every opening no higher than one foot above grade. Three things get missed. Net free area is not the size of the hole -- a louvered, screened block vent passes a fraction of its gross opening, and the fraction is what counts. The two-opening minimum is independent of area, so a small enclosure a single large vent would satisfy on square inches still fails on count, because the point is cross-flow rather than throughput. And the one foot is to the BOTTOM, so a vent in the second course of block is usually already too high. Engineered vents are certified by coverage and the square-inch rule does not apply to them at all.

non-engineered openings: required net area = 1 sq in per sq ft of enclosed area; openings required = max(2, ceil(required net area / net free area per opening)). Engineered openings: openings required = max(2, ceil(enclosed area / certified coverage per opening)), and the square-inch rule does not apply. The bottom of all openings must be no higher than 12 in above grade.
